The Case Management and Social Work teams are decentralized and embedded within the clinical areas of the hospital including the inpatient units, the emergency department, and surgical services. This team is comprised of registered nurses and masters prepared clinically licensed social workers; they work in dyads for a comprehensive multi-disciplinary approach. This high performing team is focused on patient advocacy, discharge planning, utilization review, healthcare system navigation, and connecting patients to post-acute community resources across the continuum of care.
