Ambulatory Clinics

The Ambulatory Clinics conduct telephone triage according to established protocols. Maintain active nursing appointment schedule for RN blood pressure checks, wound care, medication education and AWV. They screen patients for care opportunities, communicate and coordinate with care team on clinical issues related to patients. They are responsible for RN role in Quality Measures, participate in Professional Nursing Development by involvement in Unit Based Councils and organizational committees. They engage in projects and initiatives which facilitate standardization of care across primacy care practices.
